Automated testing establishes the exception population. It does not establish the determination. Every exception is worked by an assigned reviewer under maker-checker separation, and the determination carries its evidence.

Assigned by name and by service. Composition follows the charge population, not a general queue.
First line
Reviewers
Work the exception queue against contract rate, tolerance band and the control set for the service. Hold the correspondence: information and clarification requests, deficiency notices, adjustment notices. Record the determination and its basis.
Second line
Auditors
Test beyond the single exception. Analytical procedures, variance and trend analysis, recurring against isolated exceptions, systemic control failure against a false positive. Control rationalisation and remediation validation.
Claims
Adjusters
Assigned where coverage determination precedes charge determination. Coverage verification, liability determination, damage and loss assessment, reserve adequacy, deductible and coinsurance application, settlement authority.
Oversight
Supervisory review
Second-level and independent review, quality control and assurance, peer validation. Management certification of the reconciliation. Exception committee for items above delegated authority.
Composition
A reviewer working outside their service produces two error types at once, and neither is visible in the exception rate.
They clear charge detail a specialist would question, because they do not hold the charge basis for that service, and they raise exceptions against ordinary practice, which consumes the counterparty relationship and inflates the false-positive rate until the control is rationalised away. Bench composition follows the charge population.
